This report responds to recommendations made to the UK government following a fatal incident in 2001 when a vehicle was struck by a passenger train; this was derailed and struck in turn by a freight train. A process is outlined that should be followed to manage the accidental incursion of the railway by road vehicles. A protocol is presented to apportion responsibility and costs of improvements where public roads meet, cross or run close to railways, with a pro forma to rank risk at specific sites. Following risk prioritisation, second stage assessments may be needed. More and better data will be collected about accidental incursions. A revised barrier standard is being circulated for consultation.
